The “theranostic” approach currently revolutionizing diagnosis and treatment of disease in nuclear medicine has been enabled by enhanced availability of new radionuclides, which are crucial for this technique. The development of complementary imaging and therapeutic compounds for the diagnosis and treatment of disease requires “isotopic pairs” or “matched pairs” where radiolabels are biochemically identical or similar (respectively), making them suitable for imaging and therapy. The scandium radioisotopes offer attractive physical decay characteristics and production options leading to “true isotope pairs” (chemically identical, theranostic surrogates). Both 43Sc and 44Sc are suitable for positron emission tomography (PET) studies, while 47Sc is a β- emitter suitable for therapeutic applications with emissions.
