Method of surgical correction of severe scoliosis

The invention pertains to traumatology, orthopaedics and vertebrology and can be applicable for the surgical treatment of severe forms of scoliosis. The hooks of the fastening blocks of the two-plate endocorrector for the arches of the vertebrae are installed, starting with the thoracic to the lumbar spine, while at the ends of deformation two pairs of hooks are placed in succession on adjacent vertebrae, and subsequent hooks are installed through one vertebra. After fixing the hooks, the endocorrector plates are profiled and the dynamic fasteners are fixed to the endocorrector threaded posts at all levels of deformation, except for the apical vertebra, which is stably fixed to the plate. After contracting the endocorrector design, drainage is installed in its bed and the wound is repared. After cessation of spine growth, an incision is made in the skin along the old scar as well as the removal of the two-plate endocorrector, the removal of the articular processes of the cartilaginous layer, the decortication of the half-bones, joint and transverse processes of the vertebrae. The two-rod endocorrector with transpedicular fixation of the spine is established. The autobone-crumb obtained during decortication is placed in the endocorrector bed. The drains are installed and the wound is sewed layer-by-layer. The method makes it possible to reduce the inconvenience for patients, the disruption of the static and dynamic characteristics of the spinal column.

Method of formatting access to the retroperitoneal space organs during retroperitoneoscopic surgery

The invention pertains to medicine, more specifically to surgery. Access in retroperitoneoscopic surgery is performed on the posterior axillary line at the level of the bone-vertebral angle. Perform a cut skin and muscle layer. Perforation of the aponeurosis of the transverse abdominal muscle is carried out by a trocar with a stiletto at an angle oriented to the iliac region. The application of the method allows simplification of the formatting access and to shorten the operation time.

Method of transcutaneous transpedicular fixation of the spine

The invention pertains to medicine, more specifically to vertebrology, traumatology and orthopaedics. The projection of the vertebra to be fixed is determined using the image intensifier in the lateral and direct projections. The introduction of trepans with stilettos in the legs and the body of the vertebra is performed. The stilettos and guide the knitting needles are removed. The trepans are removed. Longitudinally, the skin, subcutaneous fat and fascia are dissected with exfoliating the muscles. The installation of soft tissue protectors is performed. The spokes channels in the peduncles and the body of the vertebra are formed to install the screws. The method allows shortening the duration of the operation and reducing the radiation load on the operating personnel. The advantages of the method are: earlier activation of the patient, hospitalization duration reducing and the probability of neurological and infectious complications decreasing.

Method of implanting the ureter into the urinary reservoir

The invention pertains to medicine, more specifically to surgery, and can be used to form ureteric-urorosurgical anastomoses after cystectomy, anterior and total pelvic eviscerations. In the prepared urinary reservoir the serous and muscular membranes are eliminated. The site is formed from the mucous membrane of the gut with a size of 3 × 1-2 cm. The distal section is made in the distal part with a length corresponding to the ureteral section. The continuous suture between the mucosa of the urinary reservoir and the stented ureter is applied. Anastomosis is covered with mobilized serous-muscle grafts. The grafts are fixed with nodal sutures. First, a suture is applied to the anterior wall of the ureter 0.5-1 cm from the anastomosis. The method makes it possible to simplify the technique of operation, to reduce the risk of postoperative insufficiency of anastomosis, vesicoureteral reflux, stenosis and stricture due to the imposing an anastomosis as a direct ureter-reservoir anastomosis and strengthening its valve mechanism.
